It is vital that you demonstrate how you meet the Essential criteria listed in the Person Specification, in order to secure an interview: How to apply - guidance for applicants | Torfaen County Borough Council As Community Support Worker in TYPSS / 15+ Team, you will be part of a professional team, dedicated to supporting and safeguarding young people through high quality social care practice which aligns with the Social Services and Well-being (Wales) Act 2014 (SSWBA) and the Wales Safeguarding Procedures. You will utilise a strength based, trauma informed approach, that promotes young people's safety, stability and wellbeing. This will be achieved through a clear focus on outcomes and positive partnership approaches with young people, families, carers, communities and other professionals. You will ensure that young people are provided with opportunities to meet their full potential, whilst having their needs met and being supported in their journey to independence. You will promote young people's participation in care planning through skilled interventions and trusting professional relationships. The Team operates a duty system to screen and process all referrals for young people aged 15 years and over. The Team is responsible for assessment and care/pathway planning for young people aged 15-18 years or 21/25 for those eligible for leaving care services. The Team supports young people to establish safety, security and belonging in their transition to adult life. Welsh Language Skills are desirable for this post but not essential.
