The service provides reablement to people living in Torfaen to support them to live independently, safely and well in their own homes or within short stay accommodation. The service is flexible and responsive and provides practical help, personal care and support in a way that is focused on the individual and is based on ‘what matters’ to them. The Planner will provide support to the Team Manager to ensure the delivery of a high quality flexible social care service in accordance with statutory and service area requirements for its provision.Duties include being responsible for the administrative functions, scheduling support workers rotas and producing statistical information for budgetary purposes. You will support the Team Manager in monitoring, reviewing and the evaluating of services provided, maintaining service provision and operational data using IT systems for service users and support workers. You will be responsible for the cost centre and annual updates and ensure regular updating of charging policy/financial information in relation to service users.
