We are currently looking for a highly motivated, solutions focused persons with an enthusiastic enabling attitude to work as Reablement Practitioners within the Torfaen Integrated Reablement Service.The service provides reablement to people living in Torfaen to support them to live independently, safely and well in their own homes or within short stay accommodation. The service is flexible and responsive.Your role as a Reablement Practitioner offers a unique opportunity to work as part of a multi-professional team.
