As a council we will seek to deliver against this through a Communities Approach. An opportunity has arisen for the appointment of a Team Lead: Complex Support within the Community and Wellbeing team. This is an exciting opportunity to lead a team with an extensive remit to provide support across Torfaen. You will provide strategic and operational leadership to the Councils Preventative Services, specifically in respect of direct delivery of support to individuals and specific case management of individuals with complex and ongoing concerns; managing demand and preventing further escalation into statutory services. You will work strategically as part of the councils Preventative Services providing impactful, targeted and appropriate support to those residents in response to a 'what matters' to them conversation. You will ensure suitable management systems and robust working practices are in place in relation to the package of support delivered by the team; for the purposes of monitoring performance and impact to manage and reducing demand, whilst meeting the individuals need. You will lead the development of the service, embedding the principles of the Communities Approach, embedding best practice and quality of service.
